Job Summary

Performs selected clinical and administrative duties that support patient care and clinical operations.

Education:  High school diploma or equivalent.  Successful completion of accredited Medical Assistant program.    

Preferred Experience/Qualifications:   Five years of experience as a Medical Assistant in a primary care setting. 

Other Qualifications:  Preferred – knowledge of clinical quality metrics, motivational interviewing and patient education techniques. Proficient use of Microsoft Excel.  

Administrative: 

Demonstrates thorough knowledge of ACP policy and procedure in performing population health duties. Population health duties will support Care Coordination team, including administrative functions of confidential medical record review, continuum of care referrals, patient education, and other outreach efforts involved in Care Coordination and Chronic Care Management.

 Technical: 

Demonstrates competency in carrying out prescribed treatment plan and monitoring patient’s condition.  Demonstrates competency in utilizing EHR software relating to patient care, and office equipment to include computer, printers, fax machine, photocopier and telephone system.  Understands basic PC windows applications including Word and Excel.
