RN Care Coordinator Team Supervisor

Come join our team! Arizona Community Physicians (ACP) is seeking experienced candidates to join our Abacus ACO as the RN Supervisor for our Care Coordination Team.  We have a team of care coordinators who provide exceptional services, and we need a hands-on supervisor with great people-aptitude and clinical patient experience to help lead them. This position will manage a team of care coordinators with responsibilities for providing clinical direction, hands-on guidance, and supervision support.

Our Abacus ACO is a wholly owned subsidiary of ACP which provides support services related to value-based care and population health programs for our physician-owned medical group consisting of 900 employees including 170 providers.  This position will work under the direction of the ACO Director and ACO Medical Director in the areas of Care Management and Population Health.

Responsibilities

Supervision

  • Manages staff, providing clinical direction and general supervision of team including hiring and training of new staff, mentoring, evaluating performance, addressing performance or team concerns, delegating work assignments effectively
  • Provides leadership in resolving concerns about service delivery, patient, and provider needs

Program service coordination and delivery

  • Helps design care coordination activities and troubleshoot problems in care coordination delivery
  • Contributes to the design of quality outcome measures and manages tracking and reporting needs
  • Reports quantifiable impact, quality of care and/or quality of life improvements as measured against program goals
  • Manages activities of programs for which responsible in accordance with ACP/Abacus policies and objectives
  • Consults with and advises Abacus Management and other administration of problems related to operation of programs; recommends changes in administrative policies to carry out program objectives more effectively
  • Establishes and maintains professional working relations with referral sources, community resources and care providers
  • Assists with the education of key providers & community stakeholders as to programs and services
  • Ensures operations conform to local, state, and federal government regulations
  • Maintains compliance with regulations governing healthcare entities and rules of accrediting bodies by monitoring operations and initiating changes where required
  • Adheres to ACP/Abacus organizational, safety, confidentiality, values, policies, and standards
  • May manage a small panel of high-risk patients
  • Serves as a back-up for the Chronic Care and Transitional Care Coordinators
  • Performs other duties as assigned

Qualifications

  • Current Register Nurse (RN) license permitting work in the State of Arizona and valid Arizona Driver’s License
  • Supervision experience preferably in healthcare, including training staff
  • Knowledge of care management techniques, tools and responses required to ensure optimal patient care
  • Knowledge of processes and procedures for patient and population management
  • Excellent presentation skills with the ability to manage questions and to work with a diverse employee population
  • Proficient use of Microsoft Office and experience with Electronic Health Records (EHR) systems, preferably EPIC
  • Skill in evaluating program and individual performance and recommending improvements
  • Ability to respond to sensitive and/or confidential inquiries or complaints from regulatory agencies, or members of the business community
  • Effective written and verbal communication skills including professionalism and courtesy
  • Ability to effectively present information to providers, management and public groups
  • Ability to interpret an extensive variety of instructions and deal with several abstract and concrete variables
  • Ability to calculate figures and compute rate, ratio, and percent and to draw and interpret bar graphs and apply basic algebraic concepts
  • Ability to define problems, collect data, establish facts, and draw valid conclusions
  • Ability to manage multiple co-existent projects, multi-task as needed
  • Skill in independent follow-through of assignments
